Policies

Appointments

We recommend arriving 10-15 minutes prior to your scheduled appointment time in order to allow for check-in prior to your scheduled time. Please bring your insurance card, and be sure to make our front desk aware of any changes to your address, phone number or personal information. Please bring any appropriate paperwork from other doctor’s visits, or forms that our office has asked you to complete.

Routine Appointments

All well child visits, routine appointments and follow up appointments will be scheduled during our normal office hours, Monday-Friday 8 a.m. – 4 p.m.

Same Day Appointments

Sometimes, you need an appointment for a sick child, and we are happy to schedule your child a same day appointment. Simply call our office and speak with one of our appointment clerks for the best time. You may request to see a particular physician. We will do our best to accommodate your request; however we may have to suggest another provider based on appointment availability.

Walk-ins

We are open seven days a week and after hours for your convenience! You may walk in at anytime with your sick child, and one of our pediatricians will see you as quickly as possible. Please note: There may be longer wait times when you walk in without an appointment. You may request a physician; however, typically you will see the first provider with an available opening.

Release of Medical Information

Your child’s health information is important and confidential. We maintain protocols to ensure the security and confidentiality of personal information. In order to release medical information, we must have a parent or guardian signed Medical Release . We will not release any medical information over the phone. When you request medical records, our office will give you a timeframe for completion.

Payment Information

Our goal is to provide the best healthcare to our patients in the most cost effective manner. We ask that you pay for your services as they are rendered which includes (but is not limited to): co-payments and/or any amount that is incurred as part of your office visit not covered by a health insurance plan. It is important for you to understand what is covered/not covered by your health insurance plan. We gladly accept new patients without health insurance; however, payment for services rendered will be expected on the date of service.

We participate in most health insurance plans, and have provided a list of commonly used plans below. Please call our office if you have any questions regarding your insurance.

Outstanding Balance

A minimum payment of $10 is expected on all accounts with a balance when a statement is received and/or at the time of the office visit. This regular monthly payment will keep your account in current status. An account balance with no payment in 30 days is considered delinquent. Continued delinquency will result in an account turn over for collection.

Medication Refills

Refills for medications may be requested through your child’s patient portal or by calling our office. It is the Doctor’s discretion as to whether or not the medication will be refilled. Please allow 48 hours for a refill request to be processed. Your pharmacy may have additional policies regarding the length of time it takes to refill a medication once the request has been received.

School Excuse

A School Excuse will only be issued if there is appropriate documentation of the child’s illness, and will be based upon the doctor’s recommendation of when to return to school.
